What affixes will a rare item get?
A rare item will get 3-6 random rare affixes (3-4 for jewels), with an equal chance for any number. Each affix has a 1/2 chance to be a prefix or a suffix, and there can be at most 3 prefixes and 3 suffixes.

NOTE: If item level is 99, affix level is always 99, regardless of quality level.

Required level
Rare item required level is usually equal to the required level of the rare affix with the highest affix level or the required level of the base item, whichever is higher. There are exceptions though, mostly for the high affix level affixes.

What affixes will an item get when crafting?
The resulting crafted item spawns with 0-3 preset crafting affixes depending on luck and shrine type used (fixed 1 or 2 preset crafting affixes for jewels), and 1-4 extra random rare affixes, based on the item level:

Item level 1-30: 40% chance of 1 affix and 20% chance each of 2, 3 or 4 affixes.
Item level 31-50: 60% chance of 2 affixes and 20% chance each of 3 or 4 affixes.
Item level 51-70: 80% chance of 3 affixes and 20% chance of 4 affixes.
Item level 71+: 100% chance of 4 affixes.

Crafted items can have at most 3 prefixes and 3 suffixes chosen from the random rare affix pool. Each random rare affix has a 1/2 chance to be a prefix or a suffix.

Required level
Crafted item required level is whichever is higher – item's required level or:

[required level of the rare affix with the highest affix level] + 10 + 3 * [number of random rare affixes]

Especially at lower levels, this formula may create items with a required level above the level of the character that crafted them. This is a feature of crafting and occurs in classic LoD as well.
